Privacy Policy
Last updated: August 2026
This policy explains how Hospice Metrics (hospicemetrics.net) handles personal information when you visit the site or contact us. Hospice Metrics compiles publicly available CMS data; this policy concerns your personal information, not the public facility data in our reports.
Who we are
Hospice Metrics is operated by Xavier Analytics LLC. You can reach us about privacy at privacy@hospicemetrics.net.
Information we collect
Information you give us
When you use the contact form, we collect the name, firm or organization, email address, topic, and message you submit. Please do not send privileged or client-identifying information through the form.
Information collected automatically
When you submit the form, our hosting provider records technical details tied to that request: your IP address, an approximate country, your browser's user-agent string, and a timestamp. These are stored alongside your message and retained for the same period. We use a bot-protection check (Cloudflare Turnstile) on the form, which processes limited technical signals to tell humans from automated abuse.
Your theme choice
If you switch between light and dark mode, your browser stores that preference locally (in localStorage). It stays on your device and is not sent to us.
What we do not do
We do not use advertising cookies or third-party analytics trackers, and we do not build advertising profiles. We do not sell personal information.
How we use your information
We use the information you provide to respond to your inquiry, to prepare and deliver any report you request, to operate and secure the site, to prevent abuse of the contact form, and to comply with legal obligations. For visitors in the EEA or UK, our legal bases are performance of a contract or steps taken at your request, our legitimate interest in operating and securing the site, and compliance with law.
Where your information is stored, and who processes it
The site is hosted on Cloudflare Pages. Contact-form submissions are stored in Cloudflare Workers KV, and form notifications are delivered by email through Cloudflare and/or our email provider, Resend. These providers act as our processors and may store or process data in the United States.
How we share information
We share personal information only with the service providers described above, and only as needed to operate the site and respond to you. We may disclose information if required by law or to protect our rights and the security of the site. Separately, the same neutral report body is provided to every party who orders one; who orders a report is treated as confidential inquiry information and is not published.
How long we keep it
We keep contact-form submissions for up to 24 months from receipt, or as long as needed to handle your inquiry and for our records, whichever is longer, then delete them. Bot-protection rate-limit counters expire automatically within an hour.
Your choices and rights
You can ask us to access, correct, or delete the personal information you submitted, or to stop contacting you, by emailing privacy@hospicemetrics.net. We will make reasonable efforts to verify your request before acting on it. Depending on where you live, you may have additional rights under laws such as the GDPR or the CCPA. We will respond consistent with applicable law.
Security
We use reasonable technical measures, including HTTPS in transit and access controls on stored submissions, to protect your information. No method of transmission or storage is completely secure.
Children
The site is intended for professional and general-audience use and is not directed to children. We do not knowingly collect personal information from children.
Changes to this policy
We may update this policy from time to time. We will change the date above when we do, and material changes will be reflected on this page.
