Original analysis of public CMS hospice data. Each note states what it counted, what the figure cannot support, and links the file it came from, so anyone can recompute it. Corrections are welcome and are recorded on the note itself.

Hospices listed by CMS in each state, 2023 to 2026
Every state with at least 25 listed hospices, across four annual CMS releases, with each state's pooled live-discharge rate and the share of its hospices that rate is computed over. Published alongside CMS's nationwide enrollment moratorium so the changes CMS describes can be checked against the public files.
The methodology sets out the definitions these notes use. A full report applies the same method to a single hospice, operator or state.
